Ltd. to secure any information regarding myself and I hereby release any person, firm, or any institution of all liabilities for any damage whatsoever from issuing such information. I further declare that the statements made by myself on this application are true, complete and correct. Any misleading / false statements given by myself on this application shall be grounds for my immediate dismissal from employment with CUCKOO International (S) Pte. Ltd., and will not persecute the company should I be dismissed.

I have applied for employment with CUCKOO International (S) Pte. Ltd. and have authorised the company to complete a thorough reference check of my past employment and education. I also agree to release from liability all persons and companies providing this information. I understand that reference information may include, but not limited to verbal and written inquiries or information about my employment preference, professional demeanor, rehire potential, dates of employment, salary and employment history.

I also understand and acknowledge that any offer of employment is conditional to CUCKOO International (S) Pte. Ltd. being completely satisfied with the information provided as a result of the reference check.
